Forms that you need so that we can assign you games are

Waiver we need every year.  If we have one you don't need to send another, and Proof of age, we only need this once and from everyone that referees. Proof of age is either a copy of your birth certificate or drivers license

You must read the Welcome Letter each year for any updates
