Forms that you need so that we can assign you games are Waiver and W-9 we need 1 for our files. If we have one you don't need to send another, and Proof of age, we only need this once and from everyone that referees. Proof of age is either a copy of your birth certificate or drivers license You must read the Informational Letter for the season(s) that you are working for To get a form click on the XX's for the format you want.
